| Procedure for the EDTA chelation therapy : |

| 1) | Contact the Clinic and make an appointment. |
| 2) | Dr. Chang will do an initial complete physical examination and screening Laboratory tests of blood and urine (to make sure the kidney function is Normal ). For heart patients EKG and/or Chest X-ray may be ordered if not done before. Please bring with you all previous medical records. The procedures of EDTA therapy will be explained to the patient at this visit. You will be advised to come back in One week to review the lab results and start the treatments. |
| 3) | The initial series consists of at least 30 treaments at the minimum. It will be given 2 to 3 times a week. A repeat of kidney function will be done one to two months after the first pretreatment test. If necessary, more treatments will be given during the initial phase. |
| 4) | After the initial phase of treatments, the maintenance phase consists of one Treatment every one to two months for indefinite period of time ideally. |
| 5) | Every therapy will last no less than 3 hours . Since the IV stand rests on rollers, patient can go to the rest room during the infusion. |
| 6) | Supplements with multivitamins and trace elements will be prescribed to take During the non-treatment days. |
| 7) | The patient should also adopt healthy life style which includes adequate exercises, balanced diets, and avoidance of smoking and excessive drinking. |

| | * Payment methods : Since insurance companies including Medicare do not pay for the EDTA therapy, therefore the patients are responsible for payment of this therapy. |
